Restricted Bequest
“I give, devise, and bequeath to [name of Aurora hospital or facility], a not-for-profit corporation existing under the laws of the state of Wisconsin, __________________ (fill in a percentage of your estate, a certain sum of dollars, or a description of particular property), and I request same be used for (describe the specific program, equipment, or other use).”
Residual Bequest
“All the rest, residue, and remainder of my estate, both real and personal, wherever located, I give, devise, and bequeath to [name of Aurora hospital or facility], a not-for-profit corporation existing under the laws of the state of Wisconsin, [to be used as directed by the Board of Directors.”] OR [“and I request same to be used for (describe the specific program, equipment, or other use).”]
